Staff Nurse provides direct and indirect nursing care pre-operatively, intra-operatively and post-operatively to client while in O.R. and P.A.C.U. to meet his physiological, psychological and sociological needs.
A. Provides Peri-Operative Nursing Care to Patient
• Visits patient a day prior to surgery, assess emotional readiness and provides psychological support and reassurance. Reports any unusual observations.
• Assist and accompany patient to operating room before, during and after the operation.
• Receives patients for surgery. Checks and identifies the patient and accomplish pre-operative checklist.
• Perform preoperative skin prepping for orthopedic patients the night prior to surgery.
• Checks the type of scheduled operation and the performing surgeon.
• Coordinates with the surgeon for any preferences in instruments to be used, supplies and equipments needed.
• Prepares the operating suite, instruments, packs and equipments necessary for the operation.
• Assist the operating team skillfully either as scrub nurse/circulating nurse.
• Assist in positioning of the patient and the preparation of the operative site
• Maintains strict asepsis intra-operatively.
• Assumes responsibility for correct sponge, needles and instruments count, certifies and documents properly.
• Maintains accurate documentation with the proceedings of patient care given.
• Maintains proper functioning of equipments used (e.g. Anesthesia machine, pulse oxymeter, electrocautery machine, suction machine, etc.) so as to provide the patient and operating team with optimal efficiency.
• Be concerned all the time with the patient’s safety, welfare, and comfort
• Coordinates with other departments (Laboratory, X-Ray, Cardiology, Pulmonary) as the needs arises.
• Checks vital signs, dressings, tubes and output before transferring patient to P.A.C.U. or Ward for proper endorsement and documentation.
• Transport or transfer patient safely and makes proper and clear endorsement.
• Responsible for preservation, labeling, documenting any tissues or specimen taken from the patient.
B. Provides Nursing Care to Patient in the Post-Anethesia Care Unit (P.A.C.U.)
• Receives patient from O.R. nurse for continuity of care.
• Assesses patient’s condition, monitors vital signs until stable.
• Assess post-operative site, drainage, output and fluids and refer accordingly.
• Provides actual nursing care needed.
• Reassures patient as consciousness is regained. Orients patient to date, time and place.
• Monitors fluid therapy and blood transfusion, outputs and drainage and documents.
• Refers any abnormalities or impending complications accordingly.
• Checks proper functioning of gadgets if connected to any.
• Endorses patient to Ward with proper documentation.
C. Unit Management
• Participates in disinfection of O.R. making sure that the unit is mechanically sterile.
• Controls the physical and emotional atmosphere in the rooms allowing team members to concentrate on the tasks without distraction.
• Identifies reports and follow-up needed repairs.
• Participates in staff meetings, problems in the unit, policies to be developed as necessary.
• Performs individual assignments efficiently (e.g. OR census, instruments and equipments inventory, consumable stocks and pharmacy inventory, control of narcotics, etc.)
• Performs On-Call Duties (24 hours stay in the OR) and Sunday call.
• Participates in disinfection of rooms, instruments, and equipments used during the day.
• Refills stocks, medicines, fluids used and checks machines and equipments in all the rooms making sure of its good condition for the next day or any emergency cases.
• Checks and allocates packs and instruments to be used for the next day’s scheduled operations.
• Makes compilation and recording of the day’s OR cases.
• Submits complete report of the day’s operations and next day’s operations to Nursing Service Office in duplicate copy.
• Checks and returns all sterile instruments and packs from Central Supply to the sterile room.
